Reread a server's SSH target on every snapshot

The target was cached for the worker's life, and the worker now runs until Orca quits, so an edited target or a re-paired server kept SSH going to the old address. Look it up each time: one Orca CLI call and a read-only query, beside the several a server refresh already makes.
