fix(trail): place the update notice doc after the selector rules; split update

The notice sentence landed between the change-selector sentences, so "It cannot be combined with --branch" read as being about update. Move it after the no-selector rule. Test the no-selector path, the one agents run most. Move patch and assignee building out of the update command, whose cognitive complexity the notice had raised.
