fix(doctor): refuse a bundle only when a user rule may be missing

Refuse when a skipped pack file or an inline/pack regex that fails to compile drops a rule the user wrote, which the previous check missed. Do not refuse when settings fail to load without declaring redaction (an unknown key from a newer CLI): packs now still load in that case and the bundle keeps the built-ins instead of steering the user to --raw. Name the custom rules in the bundle help.
