pre-push: warn about an old hook line with Entire's hooks off too

The outer-push check runs with Entire disabled for the branch, but the old-hook warning ran only from prePush. An old hook passes no ref list, so a --mirror push sent held checkpoints with nothing said. The check now warns itself when the ref list is unknown, so every caller gets it.
